The brakes have been triggered multiple times, but the evasive maneuver only activated once.

There is an avoidance feature, but it hasn't been triggered before. Not sure what's going on, the option is there in the settings.

Why do I only get collision warnings, even though I've enabled active braking in the settings, and still have to brake myself?

No, that technology didn't exist back then. There was active braking.
