Why does my car's radiator show high temperature after driving for a while? Does anyone know the reason?

I've been having a headache with my Volvo S80 2.5T. After replacing the radiator, the upper hose has come off three times, and then the lower hose came off once. Does anyone know what could be the reason? The water temperature has been normal, no overheating, and the fan is working fine.
